Summary of Benefits for EOC

New Employees - Eligible employees hired after the effective date of the plan will become eligible for insurance after completing the waiting period specified in the policy.

Eligibility - Regular, seasonal and limited term employees.

Actively at Work - Employees must be actively at work on a full-time basis in order to become insured. Employees not actively at work on their effective date will become eligible for insurance after completing the waiting period specified in the policy unless otherwise noted on the schedule of benefits.

Age Reductions - When the employee reaches the age shown below, the original amount of Employee Life Insurance will be reduced by the applicable percentage. If the employee reaches one of these ages while insured, this reduction will not apply until First of the month following birthday.

Termination - Benefits terminate at retirement. The Actively at Work requirement will not apply to this provision.

Benefit

The full benefit amount is payable in the event of death from any cause.

Beneficiary

Each employee may name the beneficiary he or she desires and may change beneficiaries at any time by filing a written request.

Employee Accidental Death & Dismemberment

Benefit Summary
An Accidental Death and Dismemberment Insurance benefit is payable under these conditions

1. The death is a result of the employee's accidental injury; and
2. The injury occurred while the employee was insured by this coverage; and
3. The death or permanent loss of a hand, foot or sight occurred within 180 days of the injury.

Loss of a hand means severance at or above the wrist. Loss of a foot means severance at or above the ankle.

Loss of sight means total loss of sight, which cannot be recovered.

A surgically reattached hand or foot will be deemed a "permanent loss" if, 12 months after reattachment, the limb has regained less than 50% of its normal function.
