Benefit Summary
An Accidental Death and Dismemberment Insurance benefit is payable under these
conditions
Benefit Amounts1. The death is a result of the employee's accidental injury; and
2. The injury occurred while the employee was insured by this coverage; and
3. The death or permanent loss of a hand, foot or sight occurred within 180 days of the injury.

Loss of a hand means severance at or above the wrist. Loss of a foot means severance at or above the ankle.
Loss of sight means total loss of sight, which cannot be recovered.
A surgically reattached hand or foot will be deemed a "permanent
loss" if, 12 months after reattachment, the limb has regained
less than 50% of its normal function.
